The survey was carried out with the InfraRed Camera (IRC) onboard AKARI which has a comprehensive mid-IR wavelength coverage in nine photometric bands at 2-24 micron. For mid-IR source extraction we used a detection image while for near-IR source detection we used optical to near-IR ground-based catalogue which is based on CFHT/MegaCam z', CFHT/WIRCam Ks and Subaru/Scam z' band detection. Here we present an AKARI source with the identification from the ground-based catalogue. For objects with multiple counterparts, all of these were listed in the catalogue with an upper limit for the AKARI flux. The magnitudes are given in AB system. If it has a counterpart from S-Cam z', neither MegaCam z' nor WIRCam Ks, the coordinates were derived from S-Cam z'. Otherwise, they were from AKARI MIR detection. Note (2): Z, K or S indicates the referred ground-based catalogue is based on MegaCam z', WIRCam Ks, or S-cam z' band catalogue, respectively. Note (3): MegaCam z' band catalogue IDs from Oi et al. (in prep.). No IDs are given for either WIRCam Ks or S-Cam z' band catalogues. Note (4): The same ID might be given to different objects, when multiple counterparts are found in the ground-based catalogue. Note (5): No coordinates based only on the NIR bands are available. MIR objects located at a region centred at (RA=269.60858, Dec=66.62320) within the radius of 5 arcmin were excluded in order to avoid the scattered light from NGC6543 (Cat's Eye Nebula). Note (6): Flag is from the ground-based catalogue as follows: 0 = no problem. 1 = multiple counterparts are found for the base-band(i.e. MegaCam z' when GRDBAND=Z or WIRCam Ks when GRDBAND=K ) source when ground-based catalogues were created. The magnitude is summed of all the candidate magnitudes but S-cam. 2 = multiple base-band sources have one common counterpart in other band. -1 = no detection. Note (7): S-cam catalogue is created using SExtractor's dual mode, based on z' band. All flags are set as 0, as far as the source is detected in the band. Note (8): Flag indicates whether the object is blended or not as follows: 0 = not be blended. 1 = blended, i.e., there are multiple counterparts in the search radius. -1 = no detection Note (9): Stellarity information derived from MegaCam z' band catalogue. This column is available only for objects with the MegaCam z' band counterparts. Note (10): Angular separation from MIR objects to the counterparts in the ground-based catalogue. This column is available only for objects with the single counterpart.
